TP-1017 - Request to have additional income tax withheld at source

Use this form if you are an employee and would like to request that your employer

• Increase the amount of income tax normally withheld at source;
• Change the amount of additional income tax withheld as requested on a previously filed TP-1017-V form; or
• Stop the withholding of an additional amount of income tax as requested on a previously filed TP-1017-V form.

Your request will remain in force until you cancel or amend it by submitting a new duly completed TP-1017-V form to your employer or payer.
