Disability tax credit certificate
Instructions
The form includes an application section (Part A), and a
certification section (Part B). Both sections of the form must be
completed.
Individuals who have a severe and prolonged impairment in
mental or physical functions, or their representative, must use
Form T2201 to apply for the disability amount by completing Part A
of the form.
Qualified practitioners must use Form T2201 to certify the
effects of the impairment by completing Part B of the form.
