CPT16 - Application For Exemption From Coverage Under the CPP

You must complete this form if you wish to apply for exemption from coverage under the Canada Pension Plan (CPP) on account of your religious beliefs. In order for your application to be valid, this form must be received by the Canada Revenue Agency no later than December 31 of the year for which you are applying.
