• T4PS/RL-25 - Employer and Trustee Information

Use this data entry page to manage the employer and trustee information to be displayed on the federal T4PS slip(s) and summary and on the Quebec RL-25 slip(s) and summary. The T4PS summary specifically requires the employer's name and address even when the trustee of the plan is completing the forms.

Note that the address of the plan's trustee is not required if you are completing the T4PS slip(s) and summary. However, the name of the trustee should be entered if you are completing the T4PS summary.
