Why have I not been granted authorization or power of attorney after having successfully transmitted the MR-69 Form electronically?
When electronically submitting the MR-69 Form, please ensure that you are attaching the proper PDF Form. For example, if you are completing a 2019 MR-69 electronic submission, you must attach a signed 2019 MR-69 PDF Form (not a 2018 MR-69 PDF Form). In addition, the attached MR-69 PDF Form must have Box 60 ticked and be properly scanned (i.e. the corners of the MR-69 Form must be fully displayed).
