Medical expenses - Travel expenses - Quebec
For Quebec residents, with respect to medical expenses for care
not provided in your area, they can be claimed if they are
incurred after June 30, 2016, within 200 kilometres of your
home.

You must have the TP-752.0.13.1 completed by the attending
physician of the person who received the medical care.

To claim for travel expenses, please follow these steps:
1. On the "Left-side menu of the Interview tab", select
the "Medical, disability, caregiver".
2. On the right-hand side of the screen, choose the "Medical
expenses" option.
3. On the page entitled "Medical expenses", go to
"Expenses for medical services not available within 200
kilometres of your area", on the following line enter a short
description, and in the field on your right, enter the amount of
your travel expenses.
4. If you incurred medical expenses, enter the cost in the
"Medical Expenses" section.
5. You must complete form TP-752.0.13.1. If you are claiming for
travel expenses, complete Parts 1, 2 and 8. Parts 3 and 4 must be
completed by the attending physician.
However, if you are claiming an amount for moving expenses,
complete Parts 1, 2, 5, and 8. Part 6 must be signed by a person
authorized by the health facility, while Part 7 must be signed by
the attending physician.
We recommend that you keep all your receipts and keep a record
of expenses you incurred.
Note: Please keep the TP-752.0.13.1 form in your files
because Revenu Québec will probably request that you submit it for
verification.
The program will carry over the amounts on line 332 00
of federal Step 5 and lines 378 and 381 of the Quebec
return.
