T4 slip - Box 85 - Employee-paid premiums for
private health service plan
You paid premiums to a private health insurance plan to increase
your coverage of the health insurance plan paid by your
employer.
This premium will be entered in box 85 of your T4 slip. For
residents of Quebec, it will be entered in box 235 of the RL-1.
To enter this amount, please follow the procedure below:
1. On the page for "T4 - Statement of Remuneration Paid
(RL-1) With QPP contributions", scroll down to the bottom of
this page and in the «Other information" section.
2. Chose the line "Employee-paid premiums for private health
services plans (RL-1 box 235)"and in the box to the right enter
the amount paid.
This private insurance premium will be carried over by the program
to your federal medical expenses on line 33099 of Step 5 of the
federal return and, for residents of Quebec, to your medical
expenses on line 36 of Quebec Schedule B
